Supporting Wabanaki Learners in Hopeful Transitions

Objective:

Supports culturally responsive transition planning for First Nation learners across key stages of their educational journey. The document adapts the provincial Hopeful Transitions model to reflect First Nation perspectives, values, and community strengths, with a focus on helping learners plan confidently for life during and after K–12 schooling.

The resource emphasizes collaboration between schools, First Nation communities, families, and service providers to ensure transitions are equitable, strengths‑based, and rooted in identity, culture, and belonging.
