Overview: This support will provide information on successful transitions. The Guides can be used by educators, school counsellors, resource educators, and administrators in furthering their understanding around career and transition. Further, these guides can be shared with learners and/or their families in an effort to support their personalized planning.
Outcomes:
1. Transition planning is individualized for each learner.
2. There are supports available for learners that may require additional information.
3. The Guides can benefit everyone in their understanding of successful transitions.
