Hopeful Transitions is a Team Effort!

To ensure that each learner is actively engaged in career pathway transition planning, all school-based roles play a part. Together, each educational role will work on goal(s) aimed at improving career pathway transition planning. By combining efforts across all school-based roles, schools will be able to support each learner in planning for the future.  Tools such as a school staff survey and descriptions of each school-based role are provided.  We invite you to work with your school district’s Career Transition Coach to access supports and resources.
